V11.3.10 is marked as compatible with foundry v12.
V11.3.x works with legacy transferral true or false. In particular it works with dnd5e 3.0 and dnd5e 2.4.x.

This module is a temporary solution to Active Effects being removed from actors as time passes and they expire.
(It's been a temporary solution since foundry v7, so probably will hang around for a while).

This module will certainly be deprecated in a future release of Foundry and will be removed at that point.
